Cerebral venous sinus thrombosis is an uncommon disorder and difficult to diagnose because of variable clinical symptoms. A 61-year old man was admitted to our hospital with dull headache and progressive cognitive decline. Brain computed tomography (CT) and Magnetic resonance image (MRI) showed cerebral deep vein thromobus with dilated medullary and subependymal veins which were prominently observed on gradient echo image. Antiphopholipid antibodies were identified on blood test. After treatment with anticoagluation, thrombus was disappeared and the patient nearly recovered. MRI with gradient echo image is essential for early diagnosis of cerebral deep vein thrombosis.
